Summary:

A variety of tests are available to arrive at the proper diagnosis of oral and perioral diseases, which may be extremely important if further treatment is indicated. When oral cancer is suspected the main diagnostic tool remains the histopathologic examination of a biopsy specimen. The value of cytologic examination, whether obtained by exfoliation of cells or by a brush technique, is somewhat questionable. A positive result of such examination needs to be confirmed by a biopsy. The same applies to the use of toluidine blue stain. The presence or absence and, if present, the degree of epithelial dysplasia as observed in a biopsy is still the most reliable predictor of possible future malignant transformation of oral leukoplakie in spite of recently reported, promising results with DNA-ploidy measurements. A salivary gland biopsy, whether taken from the parotid gland or from the accessory gland of the lower lip, plays a valuable role in establishing a diagnosis of Sjögren’s syndrome. Computertomography (CT) and Magnetic Resonance Imaging (MRI) have become important imaging tools in disease processes in the bones and the soft tissues.

The aim of this prospective randomized clinical trial was to evaluate and compare a set of clinical items and satisfaction of a group of edentulous patients during a ten-year follow-up. They were treated according to one of the following modalities: 61 patients with a mandibular overdenture on two implants (IMP-group), 60 patients with conventional complete dentures (VP-group) and 28 patients with a complete denture after preprosthetic surgery (MVP-group). It can be concluded that enosseous implants, serving as retention for a mandibular overdenture, have a high survival rate after ten years of follow-up (93%). The mean satisfaction score of the VP-group was lower than that of the IMP-group. The mean satisfaction score of the VP-group was lower than that of the IMP-group.

The aim of this study was to evaluate a secondary preventive ergonomic programme to reduce musculoskeletal disorders among dentists. Over 50% of the 57 dentists did implement the recommendations fully or nearly fully, and 40% did it partially. Barriers for a change included changing old routines and financial aspects. Important facilitators were motivation, pain and tailormade advice. The dentists did see a clear relation between the implementation of recommendations and a reduction in musculoskeletal disorders. The study showed that even in a highly motivated group recommendations to reduce musculoskeletal disorders will often be partially implemented. Improving the understanding of barriers and facilitators may result in a class of guidelines dealing with implementation issues leading to more effective prevention.

In 1995 in the Netherlands the reimbursement system for dental healtcare for the youth is partly limited, while the reimbursements of the costs for adults stopped at all. An evaluation of oral health care in The Netherlands since the alterations made in the reimbursement system shows no further improvement in the development of oral health among young people, especially among the younger age groups. The teeth of young persons up to about ten years might even be deteriorating. The upward trend in the percentage of regular visitors to the dentist did not continue after 1995. This is noticeable because the downward trend in the percentage of people without any teeth has continued. With respect to the insurability of oral health care, it is clear that people with lower incomes choose relatively often for a limited supplementary insurance. Costly treatment and calamities are hardly reimbursed. Dental treatment has become less accessible for people with a low household income.

Summary:
A 20-year-old man presented with multiple intra-oral fistulae in the anterior part of the mandible. The patient was diagnosed with secondary osteomyelitis. Microbiologic survey revealed positive cultures for Staphylococcus aureus, Haemophilus para-influenzae and alpha-haemolytic streptococci. After sequestrectomy and decortication, the mobile teeth were splinted. The patient was treated with intravenous antibiotics, followed by long-term oral antibiotics. Complete healing was achieved.

Summary:
New techniques and changing requirements on the internet forced a complete restyling of the site of the Dutch Journal of Dentistry. Also a growing number of medical and dental journals are nowadays offering full text articles on their websites. The Dutch Journal of Dentistry wants to keep pace with these developments. For the time being, some sections are under reservation to subscribers only. They will be fully available in due time. Released sections and all other sections of the journal are offered on the internet in full text and will be published together with the journal. In this article a description of the new website is presented.
